• I have read and understand the membership policies and guidelines on the membership handout. I hereby agree to pay Silver Coast Winery the initiation fee ($26) and the quarterly membership fee ($36), including any applicable sales tax.

Credit Card Authorization. I hereby request that all quarterly charges and fees be billed to my credit card and hereby authorize such billing. I certify that the above-listed card is issued to me. I understand that I am obligated to keep a valid approved credit card on file and that I am responsible for any amounts that are not paid by the credit card company. I understand I may cancel my membership at any time after the first two billing cycles and before each quarterly pickup. I understand that all wine will be held for pickup for up to two years.
